Youth Support Specialist - PRN (Greater Cleveland) Primary Location: United States-OH-OHCleveland Function: Direct Support Professional (Care Givers) Ohio MENTOR Description
Provides assistance toindividuals served with living skills development, behavioral health goals andinterventions, family visitation, doctor and like appointments, and behavioraldevelopment in accordance with an Individual Treatment Plan. Work with staffand foster families to ensure that the client’s Treatment Plans are met.
ESSENTIAL JOB FUNCTIONS
To perform this jobsuccessfully, an individual must be able to satisfactorily perform eachessential function listed below.
1.Providestraining, assistance and/or supervision to individual (s) in the areas ofcommunity and social integration, and/or behavioral health skills development.
2.Once trained,functions as aQualified MentalHealth Specialist (QMHS) by:
a.Receivedtraining or education in mental health competencies and who have demonstrated,prior to or within ninety days of hire, competencies in basic mental healthskills along with competencies established by the agency, and who are nototherwise designated as providers or supervisors, and who are not required toperform duties covered under the scope of practice according to Ohioprofessional licensure.
b.Basic mental health competencies for QMHSs shallinclude, at a minimum:
i.An understanding of mental illness,psychiatric symptoms, and impact on functioning and behavior.
ii.Howto therapeutically engage a mentally ill person.
iv.Crisis response procedures.
v.An understanding of the community mentalhealth system.
vii.Understandinghow his/her behavior can impact the behavior of individuals with mentalillness.
3.Establishes atherapeutic relationship with the individual receiving services to ensureeffective guidance, support and service delivery. Ensures objectives are met inaccordance with the Individual Treatment Plan
4.Providestransportation for individual (s) receiving services to planned and/ornecessary activities and appointments including operating vans and/or mini vansas required.
5.Maintains currentprogress and service notes. Completes and submits required documentation todirect supervisor in accordance with any applicable policies and deadlines.
6.Provides directsupervisor with verbal feedback and data relevant to the individual’s services,Service Plan or administrative policy.
7.Attends trainingand other meetings as required.
8.Reports incidentsof maltreatment, medical and behavioral concerns in accordance with MENTORpolicy.
9.Maintainsconfidentiality, respects human rights and practices universal precautions inaccordance with The MENTOR Network policy
10.Performs other related duties and activities asrequired.
